Job Description
We're urgently hiring a Remote LVN/LPN Nurse Coordinator to support patients starting and staying on specialty therapies. This role involves reaching out to patients to ensure they adhere to their prescribed therapy, addressing any issues, and facilitating communication with caregivers or charge nurses in nursing homes. The position requires handling both inbound and outbound calls, with an average of 50 calls per day and a goal of 70 calls. This role is ideal for nurses who have experience managing high call volumes while maintaining exceptional accuracy in a structured remote setting.
In this role, you will:
- Conduct telephonic outreach to patients to ensure adherence to therapy.
- Communicate effectively with patients, caregivers, and nursing home staff.
- Ensure strict documentation and adhere to templates, undergoing weekly and daily audits.
- Maintain professionalism and a high level of accuracy, with a 98% accuracy target.
- Identify and address issues such as missed doses or incorrect medication use.
- Adapt to feedback and be receptive to quality assurance reviews.
Essential Skills - Multistate/Compact Licensed Practical Nurse (LPN) certification.
- Experience in healthcare and utilization management.
- Proficiency in case management and telephonic communication.
- Strong computer literacy and attention to detail.
Job Type & Location This is a Contract to Hire position based out of Dallas, TX.
Pay and Benefits The pay range for this position is $29.00 - $29.00/hr.
