What You Will Do - Essential Functions

The Care Coordinator plays a vital, patient-centered support role by ensuring smooth non-clinical care coordination, efficient administrative operations, and accurate health information management to enable high-quality care for individuals with chronic and complex conditions.
  • Non-Clinical Care Coordination & Patient Support
    • Coordinate non-clinical components of patient care plans, including referrals, DME follow-up, and appointment scheduling.
    • Serve as a patient and family resource by answering routine questions, reinforcing care instructions, and escalating clinical concerns appropriately.
  • Administrative & Operational Excellence

    • Act as a primary point of contact by managing incoming calls, faxes, correspondence, and general inquiries to support seamless care delivery.
    • Provide administrative support through accurate data entry, workflow assistance, and special projects that enhance program operations and patient outcomes.
  • Health Information & Documentation Managemen
    • Collect, organize, reconcile, and ensure availability of external medical records critical to patient care.
    • Maintain accurate, compliant documentation in the electronic health record to support care coordination, chronic care management, and regulatory requirements.
Who You Are

Required Qualifications:
  • High School diploma
  • 1+ year of experience in healthcare, care coordination, medical office, or patient support role
  • Strong administrative and organizational skills with the ability to manage multiple priorities in a fast-paced environment
  • Demonstrated ability to communicate clearly and professionally with patients, families, and healthcare team members
  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Working Conditions:
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
  • Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
  • A quiet workspace with minimal background noise for calls.
