Job Description Job Description Promise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living.Key ResponsibilitiesManage a caseload of youth and provide ongoing support servicesDevelop and implement individualized service/treatment plansConduct assessments and monitor client progress regularlyAdvocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)Maintain accurate and up-to-date case documentation and reportsProvide guidance, mentorship, and crisis intervention when neededFacilitate life skills development (education, employment readiness, coping skills)QualificationsExperience in social work, human services, or working with youthExperience with at-risk youth preferredStrong communication and organizational skillsAbility to build trust and maintain professional boundariesCompassionate, dependable, and team-orientedAbility to handle sensitive situations with professionalismWork EnvironmentYouth-centered, supportive environmentCollaborative team settingOpportunity to make a meaningful impact in the lives of youth
