Early Intervention Service Coordinator

This Time-Limted (14) position will provide coaching, training, and guidance to EI Service Coordinators on evidence-based practices and documenting progress on implementation. Serve as an expert EI Service Coordinator: assist families of infants and toddlers with disabilities in accessing health and developmental services and make referrals to providers for needed services; provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention and other services under Part C of IDEA, including evaluations and assessments. Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP) and ensure appropriate services are being provided; inform families of their rights and procedures for safeguarding their rights and maintain early intervention records for accuracy and completeness. Assist families in achieving their goals, including understanding their child's strengths, abilities, and special needs and how to help their child develop and learn. Facilitate the development of a transition plan to preschool, school, or other appropriate services; and conduct outreach into the community to ensure all infants and toddlers with disabilities are identified and participate with community stakeholders to enhance the early intervention system.
